Description

As a home health Physical Therapist with BAYADA, you will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them remain independent. You will be part of a multi-disciplinary team, performing diagnostic tests, developing care plans, and educating clients and families.

Requirements

• Current NJ Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an APTA-approved program
• Ability to work independently and manage time

Responsibilities

• Perform home visits to clients in designated territories
• Conduct diagnostic tests and measurements (mobility, balance, strength, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate and instruct clients and family members on rehabilitative care
• Document observations and interventions using a touch pad tablet
