💼 About This Role

You'll provide home health physical therapy to adult and geriatric patients in independent and assisted living communities, as part of a multi-disciplinary team. You'll help clients improve functional independence and live safely at home with dignity.

🎯 What You'll Do

• Perform diagnostic tests and measurements such as mobility, strength, and balance assessments.
• Develop and implement individualized care plans including manual therapy and gait training.
• Educate clients and family members on rehabilitative care and activities.
• Accurately document observations and interventions using a touch pad tablet.

📋 Requirements

• Current NJ Physical Therapist license
• Graduation from an APTA-approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
