Description

You will provide 1:1 home health visits to clients in Medina County, focusing on Med/Surg and wound care. This role involves following care plans, administering medication, performing treatments, and documenting using a touch pad tablet, with flexible scheduling and strong support.

Requirements

• Active Registered Nurse license in Ohio
• Minimum 1 year recent Med/Surg clinical experience
• Graduation from accredited nursing program
• Ability to travel to client homes
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Follow designated care plans in accordance with patient needs
• Make home visits and perform assigned duties including medication administration, wound care, treatments
• Monitor client conditions and report changes to Clinical Manager
• Perform client assessments and case management coordination
• Document observations and interventions using electronic medical records
