You'll join a top-rated home health agency providing skilled speech therapy to patients in their homes. You'll evaluate and treat speech, language, cognitive, and fluency disorders while collaborating with an interdisciplinary care team.

🎯 What You'll Do

• Provide skilled speech therapy services to patients in their homes
• Evaluate and treat speech, language, cognitive, and swallowing disorders
• Serve as educator and patient advocate on interdisciplinary team
• Document patient progress and coordinate care with case manager

📋 Requirements

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of homecare regulatory guidelines preferred

✨ Nice to Have

• Experience in home health care highly preferred
• Experience with high acuity patients a plus
• Case Management fundamentals highly desired
