Description

As a home care Speech Language Pathologist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to adult and geriatric patients in their homes, helping them remain independent.

Requirements

• Current license as a Speech Language Pathologist in South Carolina
• Solid computer skills; electronic medical records (EMR) experience preferred

Responsibilities

• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluations, physician orders, and recommended goals
• Educate and instruct patients and family members on rehabilitative care to promote health and independence
• Accurately document patient care observations, interventions, and evaluations using a touch pad tablet
• Participate in multidisciplinary care plan as appropriate
