Description

In this role, you will directly engage with individuals who have high medical needs, serious mental illness, or substance use disorder, helping them access whole-person care. You'll work remotely with 2-3 community visits per month, building trust and coordinating clinical, behavioral, and social support to improve health outcomes.

Requirements

• Bilingual in Spanish and English
• Strong interpersonal and empathic communication skills
• Ability to work remotely and travel locally 2-3 times per month
• Experience with underserved populations or community health preferred
• Highly motivated and mission-driven

Responsibilities

• Outreach and engage individuals with serious mental illness, substance use disorder, homelessness, or high medical needs
• Build trust and relationships to connect patients to whole-person care
• Coordinate clinical, behavioral, and social support services
• Conduct 2-3 community visits per month near your city
• Collaborate with interdisciplinary care team
