💼 About This Role

You'll provide skilled speech therapy services to patients in their homes, evaluating and treating speech, language, cognitive, and swallowing disorders. You'll partner with case managers and the interdisciplinary team to improve patient outcomes and reduce hospital readmissions.

🎯 What You'll Do

• Evaluate and treat speech, language, cognitive, and fluency disorders
• Provide skilled therapy to assist with breathing, speaking, and swallowing
• Educate patients and families on communication and swallowing strategies
• Collaborate with interdisciplinary care team on patient plans

📋 Requirements

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
