💼 About This Role

You'll provide hospice care as a Registered Nurse Case Manager, delivering personalized end-of-life support to patients and families in their homes. Your work ensures compassionate, whole-person care with expert symptom management and emotional guidance. This role offers flexible PRN scheduling with overnight shifts.

🎯 What You'll Do

• Complete initial and ongoing comprehensive patient assessments
• Develop and revise individualized care plans with interdisciplinary team
• Administer medications and treatments per physician orders
• Coordinate care and communicate with physicians and family

📋 Requirements

• Graduate of an accredited school of nursing
• Current RN licensure in State and CPR certification
• One (1) to two (2) years of recent acute care or home care experience
• Valid driver's license with insured automobile
