Description

You will provide compassionate home health care to adult and geriatric clients in Durham and Orange counties, performing visits to implement care plans, administer medications, treat wounds, and monitor conditions. You will document care using a touch pad tablet and coordinate with the clinical team to ensure high-quality, independent living at home.

Requirements

• Current RN license in North Carolina
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Strong interpersonal and time management skills
• Ability to travel to assigned cases

Responsibilities

• Follow designated care plans according to patient needs
• Perform home visits for medication administration, wound care, treatments, and procedures
• Monitor client conditions and report changes to clinical manager
• Execute and document doctors' orders and perform client assessments
• Perform case management and coordination
