💼 About This Role

You'll provide community-based care to vulnerable seniors as part of a mission-driven interdisciplinary team. You'll build deeper relationships with fewer patients, seeing the impact of your care daily in a value-based PACE program.

🎯 What You'll Do

• Perform comprehensive in-person assessments for each participant
• Lead interdisciplinary team to develop personalized care plans
• Coordinate discharge for acute, short-term, and long-term placement
• See average of 8 patients a day, no hospital rounds

📋 Requirements

• Successful completion of residency program accredited by ACGME
• Master's or doctorate degree in medicine
• Active state physician license without restrictions
• State CDS certification and DEA registration

✨ Nice to Have

• Board certification in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
• CPR and First Aid certification upon hire
• Experience with Medicare and Medi-Cal participants
