You will lead a multidisciplinary team in providing exceptional home health care, manage field staff, oversee case management, and ensure clinical excellence and compliance with policies. This full-time in-office role in Broomfield, CO includes occasional client visits and offers opportunities for growth and advancement.

Requirements

• Active Colorado or Compact RN license
• Home health experience preferred
• Recent Medicare Certified Home Health Care experience
• Knowledge of Medicare Conditions of Participation
• Understanding of OASIS documentation and ICD-10 coding

Responsibilities

• Direct and coordinate care by a multidisciplinary home health team
• Process incoming referrals
• Mentor clinical associates and field staff
• Facilitate new hire onboarding and orientation
• Review documentation including OASIS and HCHB, monitor quality assurance
