🎯 What You'll Do

• Conduct Start of Care (SOC) admissions using OASIS assessments
• Make home visits for follow-ups and evaluation in designated territory
• Monitor patient conditions and report changes to clinical team
• Document observations, interventions, and evaluations accurately

📋 Requirements

• Current and unrestricted RN license in California
• Current BLS/CPR certification
• Valid CA Driver's License
• Experience conducting OASIS assessments for Start of Care
