💼 About This Role

You'll evaluate and treat patients in their homes to help them live safely and independently. You'll serve as an educator and patient advocate on an interdisciplinary care team. This role offers flexible scheduling and a supportive team environment with low turnover.

🎯 What You'll Do

• Evaluate patients' home safety and functional needs
• Develop and implement treatment plans for mobility and self-care
• Educate patients and families on disease management and fall prevention
• Collaborate with case managers and interdisciplinary team members

📋 Requirements

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
