💼 About This Role

You'll oversee comprehensive care coordination for a caseload of patients, developing individualized care plans and collaborating with PCPs, therapists, and CNAs. 🎯 What You'll Do

• Manage diverse caseload ensuring quality care and coordination
• Coach and oversee Certified Nursing Assistants (CNAs)
• Develop individualized care plans based on patient assessments
• Maintain accurate electronic charting within 24-hour turnaround

📋 Requirements

• Associate's Degree in Nursing or Bachelor of Science in Nursing
• Current RN license in Colorado or Compact/Multi-state Unencumbered License
• 2 years of clinical experience as a Registered Nurse
• Reliable transportation, valid driver's license, and current auto insurance

✨ Nice to Have

• 1 year of home health experience
• Case management or supervisor experience
• Knowledge of Medicaid paid family caregiving program
