💼 About This Role

You'll provide hands-on patient care in home health settings across the Bay Area, working under a Case Manager RN to develop foundational nursing skills. Your core impact is to reduce hospital readmission rates through compassionate, high-quality care. This entry-level role offers a clear path to case management.

🎯 What You'll Do

• Provide hands-on patient care including wound care, tube management, and IV therapy
• Implement and document care plans within 24-48 hours
• Assess and educate patients and families on diagnosis and self-care
• Coordinate with interdisciplinary team (HHA, OT, PT, MSW, ST)

📋 Requirements

• One year of recent nursing experience (LVN/RN) required
• Current CA Registered Nurse License (RN)
• Current Basic Life Support (BLS) card
• Valid CA Drivers License with acceptable driving record
