💼 About This Role

You'll join a multi-disciplinary home health care team making home visits in Essex County, NJ. Your core impact is evaluating social and emotional factors and connecting clients with community resources. As a Licensed Master Social Worker (LMSW) or Licensed Independent Social Worker (LISW), you'll provide clinical social interventions to help patients remain safely at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Provide clinical social evaluations and plan appropriate interventions.
• Identify and coordinate community resource referrals for clients and families.
• Collaborate with multidisciplinary team to develop care plans.

📋 Requirements

• Licensed by the STATE Board of Social Work Examiners at LMSW or LISW level.
• Master's in Social Work from a CSWE-accredited program.
• At least 2 years of community-based experience.
