Description

As a Lead Care Manager at Pair Team, you will directly engage with individuals living with serious mental illness, substance use disorder, or high medical needs, building trust and relationships to help them access whole-person care. This role is primarily remote with 2-3 monthly in-person visits in the Torrance, CA area.

Requirements

• Bilingual in Spanish and English
• Experience working with underserved populations
• Strong interpersonal and communication skills
• Ability to work remotely and travel locally for visits
• Passion for mission-driven healthcare

Responsibilities

• Outreach and engage individuals with serious mental illness, substance use disorder, homelessness, or high medical needs
• Conduct 2-3 in-person visits per month in the community
• Build trust and relationships to facilitate whole-person care
• Coordinate with interdisciplinary care team
• Support patients in accessing clinical, behavioral, and social services
