💼 About This Role

You'll lead a team of RN Case Managers and LVNs in ensuring coordinated patient care across the Greater Sacramento Area. You'll serve as the first point of escalation, support clinical excellence through education programs, and maintain relationships with customers to improve outcomes. This role offers hybrid work with 2 days in-office and 3 days in the field, plus outstanding benefits and career growth.

🎯 What You'll Do

• Oversee field clinicians providing patient care in homes
• Coordinate training and educational programs for nursing staff
• Serve as first point of contact for patient care escalations
• Conduct bi-weekly case conferences with clinical team

📋 Requirements

• Two years management/supervisory experience in healthcare
• Current CA Registered Nurse License (RN)
• Current Basic Life Support (BLS) Card
• Valid CA Drivers License with acceptable driving record
