Description

You will lead and develop a team of Outreach and Enrollment Specialists to drive participant enrollment growth for the South LA PACE center, manage performance metrics, and coordinate with community partners to ensure a smooth intake process. This role is critical to Habitat Health's mission of enabling independent aging for older adults through comprehensive care.

Requirements

• Experience managing and developing a quota-carrying team
• Knowledge of PACE or similar programs preferred
• Strong cross-functional collaboration skills
• Ability to analyze performance data and coach team members

Responsibilities

• Manage and develop a team of 5-10 Outreach and Enrollment Specialists and Intake Coordinators across 1-2 PACE centers
• Drive prospective participant enrollment success against monthly and yearly targets
• Support sourcing and maintaining local outreach channels
• Ensure prospects meet compliance requirements (DHCS and CMS) for PACE enrollment
• Review metrics daily and support growth initiatives as part of center leadership
