Description
As a Licensed Social Worker at Blackbird Health, you will deliver in-person and telehealth therapy to children and adolescents ages 2-26 using evidence-based practices. You'll build meaningful relationships with patients and families, monitor progress, and collaborate with an interdisciplinary team to provide integrated care.
Requirements
- Licensed as a Licensed Social Worker (LSW) in Pennsylvania
- Minimum 1 year of experience working with children and adolescent patients
- Minimum 1 year of experience working in a telehealth or virtual setting
- Own phone, computer, and high-speed internet
- Commit to a minimum of 8 hours per week during core hours (after 3pm Monday through Friday)
Responsibilities
- Deliver individual in-person and telehealth therapy sessions for youth ages 2-26 using evidence-based practices
- Build therapeutic relationships with patients and families to support treatment engagement
- Monitor patient progress against care goals and adjust treatment approaches as needed
- Collaborate with healthcare professionals and families to create integrated treatment plans
- Maintain thorough patient records and documentation
