You'll coordinate the bereavement program to support families and caregivers of hospice patients for up to 13 months after a loss, ensuring timely grief counseling and follow-up services.

Responsibilities

• Coordinate assessment and delivery of grief counseling needs and services
• Provide one-to-one and group grief counseling
• Conduct ongoing follow-up with families/caregivers
