Description

In this role, you will provide psychosocial support and counseling to terminally ill patients and their families, assess their needs, coordinate community resources, and collaborate with an interdisciplinary team to create compassionate care plans.

Requirements

• Master's degree in social work from CSWE-accredited school
• Active Ohio Social Work license
• Valid driver's license and insured reliable transportation
• Minimum 1 year healthcare experience with terminal illness patients
• Hospice or home care experience preferred

Responsibilities

• Conduct initial psychosocial and bereavement risk assessments within 5 days of hospice election
• Refer patients and families to community resources and assist with financial assistance
• Provide psychosocial and anticipatory grief counseling
• Participate in interdisciplinary team meetings and care plan development
• Maintain accurate clinical documentation and assist with discharge planning
