Description

You will conduct home health visits for adult and geriatric clients in Chatham and Lee counties. You'll collaborate with a multidisciplinary team to address social and emotional factors impacting health, develop care plans, and connect clients with community resources. This role empowers clients to remain at home while receiving necessary medical care.

Requirements

• Master's in Social Work with 1 year community-based experience.
• Accreditation by the Council on Social Work Education.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assist the team in understanding social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan appropriate interventions.
• Identify appropriate community resource referrals for practical and environmental needs.
• Document observations, interventions, and evaluations using a touch pad tablet.
