💼 About This Role

You'll join a multi-disciplinary team providing skilled nursing and rehabilitative care to home health and hospice clients on Maui. Your core impact will be assisting the team in understanding social and emotional factors and connecting clients with community resources. This per diem role offers flexible scheduling and competitive pay rates.

🎯 What You'll Do

• Provide clinical social evaluations and plan interventions based on findings
• Identify community resources for client and family needs
• Advocate for clients to navigate the healthcare system
• Participate in developing the total plan of care with the team

📋 Requirements

• Master's degree in social work
• At least 2 years of community-based experience
• Accreditation by the Council on Social Work Education
• Ability to travel to cases as assigned
