🎯 What You'll Do

• Serve as attending physician for patients in assigned skilled nursing facilities.
• Collaborate with NPs/PAs to manage daily patient care and clinical supervision.
• Provide primary care including diagnosing and treating acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, and quality initiatives.

📋 Requirements

• Active medical license (MD/DO) in the U.S.
• Board certified or board eligible in Internal Medicine, Family Medicine, Emergency Medicine, or related specialty.
• Commitment to patient-centered care in post-acute settings.
