💼 About This Role

You'll provide skilled home health physical therapy to help patients regain independence and function safely in their homes. Working in targeted Solano County territories, you'll create care plans, coordinate with physicians, and supervise PTAs as needed. This role offers autonomy with a flexible schedule and dedicated administrative support to minimize paperwork burden.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans
• Help patients with mobility and regain independence
• Communicate and coordinate with physicians
• Supervise Physical Therapy Assistants (PTAs)

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
