💼 About This Role

You'll provide comprehensive, holistic care to vulnerable seniors as part of an interdisciplinary team, with an average of 8 patients per day. You'll build long-term relationships and see the impact of your care daily in a value-based PACE program. This role offers regular hours and no hospital rounds.

🎯 What You'll Do

• Perform comprehensive in-person assessments
• Lead interdisciplinary team to develop personalized care plans
• Coordinate discharge for acute, short-term, and long-term placement

📋 Requirements

• Completion of ACGME-accredited residency
• Active state physician license without restrictions
• CDS certification and DEA registration
• Enrollment eligibility for Medicare and Medi-Cal

✨ Nice to Have

• Board certification
• Geriatrics, Internal Medicine, Family Medicine, or Emergency Medicine background
• CPR and First Aid certification upon hire
