You'll serve as the central weekend contact for Kaiser referral intake and clinical escalations, ensuring seamless care coordination across our home health agency. You'll support field clinicians with urgent schedule gaps and patient reassignments, acting as the primary triage resource when weekday leadership is offline. This role offers autonomous, high-impact operational work in a fast-paced environment.

🎯 What You'll Do

• Manage Kaiser referral intake and acceptance confirmations on weekends
• Assist field nurses with urgent patient reassignments and schedule gaps
• Triage and respond to clinical escalation calls from staff and patients
• Coordinate open capacity across the field team in real time

📋 Requirements

• Active California LVN or RN license in good standing
• Minimum 2 years of clinical experience
• Valid California driver's license with acceptable driving record
• Strong clinical triage skills and independent judgment

✨ Nice to Have

• Home health experience preferred
• Prior intake, scheduling, or care coordination experience
• Proficiency with CareStitch or Kinser platforms
