💼 About This Role

You'll act as a clinical conductor for high-risk patients, using real-time remote monitoring to prevent emergencies. You'll collaborate with a dedicated NP on weekends to co-manage care and challenge the healthcare status quo.

🎯 What You'll Do

• Monitor real-time physiological alerts via proprietary portal
• Perform telemedicine visits for patient education and telehealth exercise sessions
• Act as continuous point of contact for families, coordinating with PCPs and DME providers
• Collaborate daily with NPs, physicians, and engineers on clinical dashboards and care models

📋 Requirements

• Active Registered Nurse (RN) license in good standing; multistate/compact (NLC) required
• 5+ years experience in Internal Medicine, Geriatrics, ER, or ICU
• 1+ years focused on chronic cardiopulmonary conditions (CHF/COPD)
• Proficiency with Google Workspace and ability to learn new software quickly
