Description

As a Hospice RN Case Manager, you'll manage a caseload and provide skilled nursing care to patients facing life-limiting illnesses, working with a compassionate team to develop personalized care plans that prioritize comfort and quality of life.

Requirements

• Valid Ohio Registered Nurse (RN) license
• Graduate of approved nursing program
• 2-4 years direct patient care experience in hospice
• Case management experience preferred
• Experience with end-of-life diagnosis preferred

Responsibilities

• Manage primary caseload of hospice patients
• Provide skilled professional nursing care as prescribed by physician
• Ensure compliance with Medicare Conditions of Participation and Ohio hospice laws
