Case Manager - Offender Services

Description

In this part-time role, you will support justice-involved individuals transitioning from incarceration to the workforce through the Pathway Home 6 grant. You'll provide one-on-one case management, develop reentry plans, and connect job seekers to training and support services to reduce recidivism.

Requirements

  • Strong communication, organizational, and interpersonal skills
  • Ability to collaborate with jail staff, service partners, and employers
  • Experience with case management and risk/needs assessments (e.g., LS/CMI)
  • Proactive and compassionate advocacy for job seekers
  • Ability to maintain accurate documentation and compliance with federal standards

Responsibilities

  • Provide one-on-one case management to incarcerated and recently released job seekers
  • Develop and maintain Individualized Development Plans (IDPs) linked to training, employment, and support services
  • Connect job seekers with supportive services (housing, legal aid, mental health, etc.)
  • Facilitate job readiness training and guide job placement and post-release training pathways
  • Maintain participant records and monitor WIOA indicators and recidivism metrics
