🎯 What You'll Do

• Enroll patients into RTM/CCM programs via Flagler's portal
• Make daily calls and texts to document health status
• Collect patient health information and new symptoms
• Send health pointers like stretches and meditation videos

📋 Requirements

• LPN/LVN certification
• 3+ years experience in office or remote
• Proficiency with Zoom, Slack, and online enrollment platforms
