Description

You will be an integral member of a multi-disciplinary health care team, providing rehabilitative care and skilled nursing to clients in their homes. You will perform evaluations, develop treatment plans, and educate patients and families on therapeutic interventions.

Requirements

• Current license as a Speech Language Pathologist in North Carolina
• Certificate of clinical competence in speech from ASHA or equivalent educational requirements
• Solid computer skills; prior experience with electronic medical records (EMR) preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation, physician's orders, and recommended goals
• Continually assess and revise the speech therapy care plan, participating with clinicians in multidisciplinary care
• Educate and instruct patients and family members in rehabilitative care and activities
