Description

As a Lead Care Manager at Pair Team, you will directly engage with individuals living with Serious Mental Illness, Substance Use Disorder, or experiencing homelessness, building trust to connect them with whole-person care. This role is mostly remote but requires 2-3 monthly on-site community visits near you. You'll be part of an interdisciplinary team using AI and automation to reduce administrative burden and improve outcomes for high-need patients.

Requirements

• Bilingual in English and Spanish
• Experience working with underserved populations
• Strong communication and empathy skills
• Ability to work both remotely and in community settings
• Motivated to transform primary care operations

Responsibilities

• Outreach and engage individuals with high medical needs, SMI/SUD, or experiencing homelessness
• Build trusting relationships to connect patients with whole-health care
• Coordinate with interdisciplinary care team
• Participate in monthly on-site community visits
• Contribute to closing care gaps and improving patient outcomes
