Description

In this PRN role, you will plan, organize, and direct hospice care for patients and their families, conducting comprehensive assessments and coordinating care plans to ensure comfort and dignity at end of life.

Requirements

• Experience in nursing with emphasis on community health
• Current RN license
• Ability to perform comprehensive physical assessments
• Knowledge of hospice and palliative care principles
• Strong communication and coordination skills

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments
• Develop and revise care plans based on nursing diagnosis and palliative goals
• Administer medications and treatments as prescribed
• Communicate with physicians and interdisciplinary team to coordinate care
• Participate in on-call duties as defined by policy
