You'll provide telephonic nursing support for members with cancer and complex behavioral health needs, partnering with a care team to address symptom management and clinical education. 🎯 What You'll Do

• Conduct telephonic assessments and medication reconciliation
• Monitor member progress and establish targeted support plans
• Partner with care team to address social determinants of health
• Participate in interdisciplinary case conferences

📋 Requirements

• BSN degree
• 5+ years nursing experience with 2+ years in complex behavioral health
• Multi-state unrestricted RN compact license
• Preferred oncology nursing experience
