💼 About This Role

You'll provide rehabilitative care to adult and geriatric patients in their homes, conducting evaluations and developing treatment plans for speech, language, communication, or swallowing difficulties. You'll be an integral member of a multi-disciplinary team enabling patients to remain at home with skilled nursing support. Prior home care experience is a plus but not required.

🎯 What You'll Do

• Conduct home visits for evaluations and therapy
• Develop treatment plans based on evaluations and goals
• Educate patients and families on rehabilitative care
• Document patient care using electronic medical records

📋 Requirements

• Current SLP license in Arizona
• Certificate of Clinical Competence in Speech from ASHA or equivalent
• Solid computer skills and prior EMR experience preferred
