Description

You will conduct in-home assessments and functional evaluations for older adults and high-needs populations, delivering personalized home modification recommendations and collaborating with a remote clinical team to help clients live safely and independently.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Reliable transportation and valid driver's license

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
• Maintain accurate and timely documentation through electronic system
