Description

You will provide one-on-one care to clients in their homes, with flexible scheduling and support from a dedicated clinical team. This role offers paid training, short commutes, and the chance to build meaningful relationships while delivering high-quality home health care.

Requirements

• Valid LPN license in Pennsylvania
• Ability to work flexible shifts including nights and weekends
• Reliable transportation and willingness to travel to client homes
• Strong communication and interpersonal skills
• Commitment to providing compassionate care

Responsibilities

• Provide one-on-one nursing care to clients in their homes
• Perform skilled nursing visits and interventions as per care plan
• Document patient care electronically using AlayaCare
• Communicate with clinical support team as needed
• Adhere to infection control and safety protocols
