Description

As a Physical Therapist with BAYADA Home Health Care on Oahu, you will provide skilled rehabilitative care to clients in their homes, developing and implementing individualized treatment plans to help them achieve their goals. You'll collaborate with a multi-disciplinary team and have the flexibility to create your own schedule while making a meaningful impact on clients' lives.

Requirements

• Current Physical Therapist license in Hawaii
• Graduation from a program approved by APTA or CAAHEP
• Solid computer skills; EMR experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Responsibilities

• Perform diagnostic tests and measurements for mobility, strength, and balance
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Implement wound prevention and care using evidence-based dressings and agents
• Educate clients and families on rehabilitative care and independent living activities
• Accurately document patient care management and services using electronic medical records
