💼 About This Role

You'll provide hands-on therapeutic interventions in patients' homes as part of a supportive interdisciplinary team. Your role focuses on follow-up visits to build core clinical skills while helping reduce hospital readmission rates. This entry-level position offers strong mentorship and training from leadership.

🎯 What You'll Do

• Provide gait training, balance exercises, and manual therapy to patients.
• Conduct detailed patient evaluations in the home environment.
• Document patient care accurately within 24-48 hours.
• Collaborate with the supervising PT Case Manager on care plans.

📋 Requirements

• Current California PT License unrestricted.
• Current BLS/CPR Certification.
• Valid CA Driver's License with clean driving record.
• Graduate of an accredited Physical Therapy program.

• One year of experience as a Physical Therapist.
