🎯 What You'll Do

• Develop and maintain relationships with hospitals, physicians, and facilities
• Educate referral sources on hospice eligibility and benefits
• Identify hospice candidates and coordinate referrals
• Provide education to patients and families on hospice philosophy
• Represent the hospice organization at community events

📋 Requirements

• 2–3 years of healthcare experience required
• Hospice or palliative care experience strongly preferred
• Relationship-building and communication skills
• Knowledge of hospice eligibility criteria preferred

• Associate or Bachelor’s degree preferred
• Experience with CRM systems and EMR platforms
