Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them receive the medical care they need to remain at home. As a member of a multi-disciplinary team, you will perform diagnostic tests, develop care plans, and educate clients and families.

Requirements

• Current Pennsylvania Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from an APTA or CAHEA approved program

Responsibilities

• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance, etc.)
• Implement integumentary protective interventions for wound prevention and repair
• Develop and implement individualized care plans including manual therapy, gait training, and balance exercises
• Educate and instruct clients and families on rehabilitative care for health and safety
• Document observations, interventions, and evaluations using a touch pad tablet
