💼 About This Role

You'll coordinate participant care as part of a PACE program, serving as the primary advocate and navigator to ensure care plans come to life. You'll manage transitions, communication, and documentation across multiple stakeholders and settings, enabling seniors to age in place with dignity. This role is central to building a scalable, culturally-competent care model at a VC-backed startup.

🎯 What You'll Do

• Track follow-up needs and manage care transitions across settings.
• Coordinate post-hospitalization follow-up including DME and referrals.
• Manage day-to-day communication with home health agencies and specialists.
• Prepare for care team meetings and track action items.
• Maintain accurate documentation in EHR and other platforms.

📋 Requirements

• 2-5 years of experience in healthcare coordination or similar role
• Ability to manage across multiple systems and communication channels simultaneously
• Comfort working in fast-moving, resource-constrained environments
• Fluent use of technology tools to organize workflows and communicate

• Experience evaluating or implementing new tools in healthcare
• Mandarin or Cantonese language proficiency
• Background in social work, public health, or health administration
