Responsibilities

• Deliver skilled nursing care to clients in their homes
• Provide 1:1 support for clients of all ages and acuity levels
• Maintain accurate electronic charting using Cubhub
• Collaborate with clinical team for continuity of care
• Adhere to care plans and state regulations
