🎯 What You'll Do

• Provide leadership, oversight, and performance management to RN, LVN, and MA staff
• Oversee clinic function to address chronic and acute participant needs
• Liaise with Complex Care team on smooth transitions of care
• Develop workforce plans and ensure appropriate staffing

📋 Requirements

• Graduate of an accredited school of nursing with unencumbered RN license
• Minimum of two years nursing experience in a clinical setting with frail/elderly population
• Minimum of one year supervisory experience preferred
• Experience leading in a data-driven organization

✨ Nice to Have

• Bachelor's degree (BSN) preferred
• Case management or care coordination experience preferred
• Outpatient/clinical experience preferred
