You'll lead a multidisciplinary team providing primary care to geriatric patients at a Federally Qualified Health Center. You'll have extended clinic time to address complex medical and social needs, improving quality of life for underserved seniors.

🎯 What You'll Do

• Provide primary and consultative medical care to geriatric patients.
• Lead SAGE interdisciplinary team including nurse and community health worker.
• Conduct screenings for frailty, cognitive decline, and functional limitations.
• Collaborate with specialists and interdisciplinary team members.

📋 Requirements

• Experience in caring for patients with geriatric issues such as cognitive impairment, falls, or polypharmacy.
• Strong communication and interpersonal skills with older adults.
• Commitment to providing care to underserved populations.
• Valid California medical license and DEA registration.
• Board certification in internal medicine or family medicine.

✨ Nice to Have

• Fellowship training in Geriatrics or 2+ years geriatric experience.
• Bilingual in Chinese (Mandarin or Cantonese), Vietnamese, or Asian languages.
• Proficiency with EPIC EHR.
• Experience in community health or FQHC setting.
