You'll provide skilled nursing and rehabilitative care to clients in their homes as part of a multi-disciplinary team. Your core impact is helping patients remain at home while receiving necessary medical care. You'll work on a per visit basis in Summit County with a supportive team.

🎯 What You'll Do

• Making home visits to clients in designated territories
• Administering medication, wound care, treatments, and procedures
• Monitoring clients' conditions and reporting changes
• Documenting care using electronic medical records on a tablet

📋 Requirements

• Active LPN license in Ohio
• 1+ year of recent verifiable LPN experience
• Graduation from an accredited nursing program
