As an Occupational Therapist with Jukebox Health, you will conduct in-home assessments, deliver personalized home modification recommendations, and educate clients on fall prevention. Your work will help older adults and individuals with disabilities live safely and independently, supported by a tech-first team.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
• Maintain accurate and timely documentation through electronic system
