💼 About This Role

You'll lead a caseload of high-need individuals in Pair Team's whole-person care model, focusing on Serious Mental Illness and Substance Use Disorder populations. Your work directly reduces emergency visits and hospital admissions through trust-based engagement strategies.

🎯 What You'll Do

• Maintain ongoing caseload using evidence-based engagement approaches.
• Conduct periodic telephonic and SMS outreach for timely follow-up.
• Collaborate on care issues with Enhanced Care Management team.
• Assist individuals in securing community supports and referrals.
• Meet monthly encounter metrics for health plan compliance.

📋 Requirements

• 1+ years of general work experience (Case Management preferred).
• Physically located in California with reliable transportation.
• Bilingual – English/Spanish with strong cultural fluency.
• Valid driver's license and auto liability insurance.
